  • Making things understandable and answering the patient’s questions

Collaboration with other caregivers

  • Calling on other consultants and ordering tests needed to diagnose and treat a condition
  • Consulting with the care team to develop the care plan
  • Monitoring to assure care is safe, effective and efficient

Coordination at admission and discharge

  • Contacting the patient’s primary physician, surgeon or other specialist to share information
  • Planning for the patient’s transfer out of the hospital and for appropriate follow-up care
