Personal Health Management Program Enhances Wellness


"CHI has introduced a program to help employees who have chronic health conditions improve their health and quality of life. The Personal Health Management Program, which replaces CHI's former disease management program, enables employees and spouses who are enrolled in the CHI Medical Plan and who have chronic health conditions to work with a personal nurse advocate. The nurse advocates provide customized plans, information and support to help employees manage their conditions and prevent progression to a more critical stage.

"As part of the Healthy Spirit program, we took a fresh look at disease management for CHI employees," said Allan Calonge, director of health and welfare plans for CHI. "We wanted to expand the program and help employees who have chronic health conditions be more proactive about managing them. Whenever we can enhance the wellness of employees, we enhance the overall vitality of CHI."

Working with a new partner, Carewise Health, CHI introduced the Personal Health Management Program on February 1. The program is for employees who have:

  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Coronary artery disease
  • Diabetes
  • Heart failure
  • High blood pressure
  • High cholesterol

"Working with the nurse advocates, employees can receive information and support, set realistic goals and better manage their lifestyles," said Calonge. "This also helps employees work more closely with their physicians to make good decisions about their health."

Carewise Health identifies employees who are candidates for the program based on claims data shared with strict adherence to HIPAA privacy standards. Employees may also refer themselves or their spouses to the program by calling 1-877-878-5445.

While the Personal Health Management Program is designed for employees whose health conditions have a higher level of acuity, another program to be introduced this fall will provide health and lifestyle coaching for all CHI employees. "This will be a broader program that can help any employee - even those at the peak of good health - maintain health and wellness for the long term," said Calonge.
