Memorial Home Health Reduces Rehospitalization

Memorial Hospital Home Health, Chattanooga, TN, achieved the lowest acute care rehospitalization rate in its service area, according to a Home Health Compare update from the Centers for Medicare and Medicaid Services. As of March 2009, Memorial’s rehospitalization rate was 22 percent, compared to the Tennessee average of 32 percent and the national average of 29 percent. Under Medicare’s Pay for Performance reimbursement model, slated to begin as early as January 1, 2010, approximately 20 percent of a home health agency’s reimbursement could be tied to its acute care rehospitalization rate.
